RN Qualifications/Duties Graduate of an accredited Nurse Practitioner program.
Current license to practice as an Adult Nurse Practitioner in the state of Texas Current RN license and certified as an Adult Nurse Practitioner with prescriptive authority Responsibilities of the position:
-Demonstrates contribution to chronic disease management programs, post acute care follow:
up program and wellness programs to monitor and continuously improve the quality of patient care.
-Coordinates with chronic care team including nurses and support personnel to ensure optimal patient outcomes.
-Meets with staff to coordinate and schedule patient care or discuss issues concerning patient care.
-Assists and teaches staff regarding patient care issues as needed.
-Develops and/or procures patient educational materials in collaboration with team members.
-Demonstrates awareness of entire departmental patient care needs and works with staff to ensure adequate patient care coverage.
- Acts as a resource for staff and support personnel in solving departmental issues.
-Serves as a preceptor in orientation/assessment of clinical competence with new hires as indicated.
-Assists in development of and ongoing monitoring of capital and operating budgets.
-Initiates action to meet patient and/or significant others need for information.
-Takes action to reduce/eliminate patient risks.
-Uses Chronic Care Management Model to obtain adequate data to initiate, implement, and revise plan of care to meet patient, physician, and family needs.
-Acts independently and/or in collaboration with the health team in the observation, assessment, diagnosis, intervention, evaluation, rehabilitation, care and counsel, and health teachings of persons who are ill, injured or infirm or experiencing changes in normal health processes; and in the promotion and maintenance of health or prevention of illness.
-Utilizes protocols or other written authorization promoting the exercise of professional judgment commensurate with education and experience.
-Serves as patient and staff advocate.

Estimated Salary: $20 to $28 per hour based on qualifications.
